A Program of Practical Support
After reading about Tidewell's extensive services, perhaps you're still not certain it's the right fit for your current health care needs. If so, perhaps Tidewell Transitions is a more appropriate option.
Transitions is a volunteer-based program designed to provide you with non-medical support and practical assistance. A Transitions Coordinator meets with you and your family to assess your needs, then coordinates your volunteer services and community resource information. Volunteers are specially trained, experienced and caring. They offer support, friendship and respite for patients and their families. They help with transportation, errands, food preparation, friendship and more.
The Transitions Coordinator serves as your guide, helping you identify needs, providing emotional support for decision making and pointing you toward valuable community services.
Transitions is available to anyone who has been diagnosed with an advanced illness and has a prognosis of one year or less. You can meet with the Transitions Coordinator and learn more about the program by calling (941) 552-5922 or toll free 888-202-9972.
There are no charges for services. Transitions is fully funded through Tidewell Hospice. Volunteers and staff visit clients at home, in the hospital or at an extended care facility.
