Not Ready for Hospice?
After reading about Tidewell's extensive hospice services, if you're still not certain whether or not it's the right fit for your loved one's current health care needs, perhaps Transitions is the answer.
Transitions is a volunteer-based program designed to provide you and your loved one with non-medical support and practical assistance. Once you've chosen Transitions, and your loved one's doctor has agreed, a Transitions Coordinator meets with you to assess needs, then coordinates your volunteer services and serves as your guide, providing emotional support for decision making and pointing you toward valuable community services.
Volunteers are specially trained, experienced and caring. They offer support and friendship, help with transportation, errands, food preparation and more. And for you as a caregiver, Transitions volunteers can allow you valuable time for yourself by providing respite while they sit with your loved one.
Transitions is available to anyone who has been diagnosed with an advanced illness and has a prognosis of one year or less. You can request an appointment to meet with the Transitions Coordinator and learn more about the program by calling (941) 552-5922 or toll free 888-202-9972.
There are no charges for services. Transitions is fully funded through Tidewell Hospice. Volunteers and staff visit clients at home, in the hospital or at an extended care facility.
