LPN FOR CRISIS CARE (NIGHTS)

PRN AND FULL-TIME OPPORTUNITIES AVAILABLE

Purpose: The Crisis Care (CC) Licensed Practical Nurse/Licensed Vocational Nurse (LPN) is responsible for the observation of patient/family needs and providing palliative and supportive care to the patient/family unit. As outlined in the plan of care, the CC LPN utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. The CC LPN provides emotional support during times of crisis to the patient/family unit.

Location/Work Schedule

Opportunities available in Charlotte, Desoto, Sarasota and Manatee Counties

Schedules Vary with alternating weekends

12 hour shifts – 8:00pm – 8:00am

Primary Tasks, Duties and Responsibilities:

Under the direction of RN case manager, provides clinical, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition as needed. Provides support to family/caregiver reporting psychosocial/spiritual needs to team.

     Nursing documentation displays accuracy, timeliness, legibility, professional terminology and awareness of legal implications when nursing care is delivered.
     Presents concise, pertinent oral and written information to Team; respects input from all disciplines.
     Incorporates behaviors supportive of internal Mission Statement, and organizational values.
     Submits paperwork in a timely manner, and demonstrates responsibility in work attendance.
     Performs within the framework of the philosophies, policies and procedures of Tidewell Hospice.
     Communicates accurately and completely to staff members, patients, families and supervisors; utilizes positive approach when working with other staff members.
     Attends Tidewell Hospice scheduled meetings.

Educational/Professional:

     Graduate of school of vocational nursing.
     Current, clear and active license in the State of Florida or current licensure from another State plus a temporary Florida permit.
     Ability to communicate in English, bilingual Spanish preferred.
     One year of recent health care experience required.
     Hospice/Home Health experience preferred.
     Valid State of Florida driver’s license and automobile insurance.
